You will be working closely helping and supporting disabled adults ensuring the patient receives the correct healthcare plan and maintenance.

The post is in a fully integrated Multi-disciplinary team, carrying out Care Act/Strength based Assessments, Support plans and Reviews and Mental Capacity assessments. We are currently looking for an experienced and dedicated Social Worker to work full-time within the team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adult's service.

Duties and Responsibilities:

• Assessment of risk and purposeful intervention through direct work
• Undertake assessments and help service user plan
• Develop support plans that gain commitment to solutions that drive the care required for each individual's needs.

Requirements of the Social Worker:

• hold a degree, post graduate or Professional Qualification in Social Work

• Hold full HCPC registration. A clear understanding of safeguarding is critical to this role.
• Full working driving license
