Social Worker - Older Peoples Team - Covering South Wales (Ongoing Contract)
NonStop's client is based in Wales is currently seeking a Social Worker for the Older Peoples team. In terms of locality, the team will be covering the Brecon area. As a social worker within the team you will need to carry out some face to face appointments, carrying out all of their visits on 1-2 days and carry out paperwork and teams meetings on the other days.
As a full time staff member your caseloads are capped at 25 cases. If the person has particularly complex cases this can be lowered but does not go over 25. Working at this Local Authority gives you the best of both worlds. You have the opportunity to take on a learning curve of working in a borough which covers both inner and outer city environments. You will be covering a wide range of caseloads varying in complexity developing your knowledge.
Responsibilities:
- To assess and care manage the complex needs of older people and their carers to ensure their health, well-being and social inclusion.
- To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
- To promote partnership working which supports the person centred outcomes to be delivered for older people.
- To undertake safeguarding investigations as required.
