Project Support Advisor (Contract)
JOB TITLE - Well being worker
LOCATION - Penarth with home working avaliable
SALARY - £17,500 - £22,000
JOB TYPE - Full time, Fixed term contract till March 2022
The purpose of this role is to provide direct carer support. The role will involve supporting carers in the community, undertaking the assessments and offering support when appropriate. There will be a limited budget to help carers have access to equipment and services to benefit them in their role.
RESPONSIBILITIES
- work closely with colleagues to develop well being services across Cardiff
- To collate monitoring and reporting evaluation
- develop community outreach support in Cardiff
- To develop knowledge of local services
- To identify potential new service users and hidden carers
- to undertake appropriate assessments & reviews with carers and award funds to allow carers to access equipment and services.
- provide appropriate support
- Support with the effective running of the project, setting up and maintaining systems.
IT IS ESSENTIAL
- Understand social services and well being act
- 5 GCSES
- Knowledge of commitment & understanding of the social care sector
- Use of own car
