Social Worker - Children in Need - Up to £46'000 (Permanent)

We are seeking a highly motivated and driven social worker to join our client's team. The successful candidate will be responsible for managing long-term care planning for service users. You will develop care plans that maximise independence and quality of life for service users while ensuring that all clinical and regulatory requirements are met.

You will work closely with clients, families, healthcare providers, and community resources to ensure that the highest level of care is achieved.

Responsibilities:

• Assessing service users' needs for long-term care services and developing appropriate care plans.
• Understanding and interpreting clinical guidelines and regulations in order to ensure compliance.
• Coordinating the delivery of care services to clients in various settings, including home, community, and residential care facilities.
• Regularly reviewing and updating care plans based on changes in service users' health status or needs.
• Collaborating with healthcare providers, family members, and other community resources to develop comprehensive care plans.
• Ensuring that all documentation and reporting requirements are met in a timely and accurate manner.
• Participating in quality improvement activities and providing ongoing feedback on the effectiveness of care services.
