NonStop's client is seeking a Qualified Social Workers for their Complex Care and Review team, which is a small but extremely effective team.
The team consists of a Practice Manager, 2 x Social Workers, 2 x Social Care Assessors and 1 x Occupational Therapist. The expectation is that staff work in the office 1-2 times a week and at home or out on visits the remainder of the week.
The team is unique in that it completes reviews taking a 'whole service/provider' approach. In doing so they work closely with providers and commissioners to improve the standards of current services. This includes setting clear outcome focused care plans, taking an innovative approach regarding effective use of current services, and scrutinising services to ensure commissioned support is required and being used appropriately in keeping with a person-centred and strength-based approach.
