Our client in London is looking for an Adult Social Worker to join their Community Mental Health Team, providing high-quality support and treatment to individuals with severe and enduring mental health needs.

Responsibilities:

- Conduct social care assessments and contribute to the development of care plans.

- Provide social work interventions to individuals and groups of service users with complex mental health needs.

- Work collaboratively with other professionals to deliver coordinated care and treatment.

Requirements:

- Hold a degree in social work or equivalent.

- Have a minimum of two years' post-qualifying experience in mental health social work within a community mental health team.

- Be registered with the HCPC.

- Have experience of carrying out social care assessments and completing support plans.

- Knowledge and experience of the mental health act and its application in practice.

- Experience in carrying out mental health needs assessments and implementing recommended interventions.

- Possess experience of working with people with complex mental health needs, including those with co-morbid conditions.
