We are currently seeking a highly motivated and passionate Learning Disability Social Worker to join our client's Community Health and Social Care team.

Responsibilities:

- Provide face-to-face holistic and comprehensive strength-based case management services, including care management assessments/reviews and carer assessments/reviews, risk assessments, mental capacity assessments, best interest assessments, and ordinary residence transfers.

- Manage and outcome complex Continuing Healthcare cases and disputes.

- Liaise and consult with users, carers, colleagues, and various statutory and voluntary agencies.

Requirements:

- 5 years of social work post-qualification experience.

- Social work England registration.

- Degree in social care.
