Are you a Social Worker seeking a new role in Adult Community Rehabilitation? Our client, based in London, is currently looking for someone to join their multi-disciplinary team. The team excels in providing exceptional services to clients. If you have over 2 years' experience, this is an opportunity for you to work with one of the highest-performing local authorities in the UK.

Primarily community-focused, the role necessitates flexibility in working across various locations, including the Hackney Service Centre and St. Leonards. The incumbent will collaborate closely and proactively with ACRT clinicians, employing a strengths-based methodology to address the multifaceted needs, foster self-management, and promote independence among clients grappling with long-term health conditions.

Duties will encompass participation in multidisciplinary meetings, conducting assessments, providing advice, and advocating for personalised support to realise the objectives outlined in tailored support plans. Additionally, the role involves identifying, supporting, and facilitating training opportunities across partners, fostering a unified approach to accessing partner information.

Responsibilities:

- Work collaboratively with ACRT clinicians using a strengths-based approach to manage the complex needs of clients living with long-term health conditions.

- Attend multi-disciplinary meetings and undertake assessments to identify the support required for realistic, focused, and personalised support plans.

- Support and facilitate training opportunities across both partners for a shared approach in accessing partner information.

Requirements:

- Have at least two years of post-qualification experience in adult social care.

- Possess Social Work England registration.

- Have a degree in social care or any equivalent.
